Cagrilintide is not a component of any FDA-approved drug and has not been found safe and effective, so compounding products containing cagrilintide is prohibited under federal law; the FDA has separately proposed excluding semaglutide from the 503B bulks list (part of the broader 2025-2026 wind-down of compounded GLP-1 medicines). Neither GLP-1 receptor agonists nor amylin analogues are specifically named on the current WADA Prohibited List, but athletes should verify status with their governing body.

CagriSema is an investigational fixed-dose combination of cagrilintide, a long-acting amylin analogue, and semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist, developed by Novo Nordisk for chronic weight management. The two components act through complementary appetite-regulating pathways: semaglutide engages GLP-1 signaling in vagal and hypothalamic circuits, while cagrilintide acts on amylin receptors in the area postrema, together reducing hunger and promoting satiety. It has been evaluated in the large phase 3 REDEFINE program in adults with overweight or obesity, with and without type 2 diabetes, where it produced substantial mean body-weight reductions over 68 weeks alongside improvements in glycemic and cardiometabolic markers. Because both components are studied human peptides assessed in randomized controlled trials, the research base is relatively robust, though the combination is not yet an approved drug. Novo Nordisk submitted a New Drug Application to the U.S. FDA in December 2025, with regulatory review anticipated during 2026.
